Abstract
Medical errors do happen in health care service delivery, like errors in any service industry or manufacturing industry. But, how serious are medical errors?. The seminal article "To Err is Human, Building a Safer Health System" (IOM, 1999) reported that preventable medical errors accounted for the death at least 44,000 people, and perhaps as many as 98,000 people in U.S. hospitals each year. Medical errors could lead to Adverse Events (AE), many of which are preventable. AEs cause concerns to all the three major stakeholders in health service delivery : namely, the management, service providers, and the community. AEs increase the financial burdent to the hospital management resulting from extended length of stay of AE burden to the hospital management resulting from extended length of stay of AE patients. AEs indicate poor quality of medical service delivery by the healthcare professionals. AEs may cause patient injuries with or without disabilities and thereby project a poor image of the hospitals in the community. Since a large number of medical errors can be prevented, hospitals must analyse the causes of medical errors and implement preventative programs to reduce their occurrence. Minimizing medical errors is central to the provision of good quality medical services.
